What We Do
Horizon San Gabriel Valley is a brand-new 62-bed Recuperative Care Center.
Our mission is to end the cycle of homelessness by providing quality care in a safe and comfortable environment for those who need further support.
Horizon is proud to offer a range of services. We ensure residents are housed, recover from post-acute care, take medication, and attend all follow-up appointments. At the same time, we help residents obtain long-term housing and set them up for success.

Dedicated Care Manager
Medication Oversight

Connection to Mental Health and Substance Abuse Professionals
